1. 해당 요양기관의 종류와 그 요양기관 대표자의 면허번호
2. 의료기관의 개설자가 법인인 경우에는 의료기관의 장의 성명
3. 그 밖에 다른 요양기관과의 구별을 위하여 법 제100조제2항에 따른 건강보험공표심의위원회(이하 "공표심의위원회"라 한다)가 필요하다고 인정하는 사항
