법 제108조에 따른 심사청구는 다음의 사항을 적은 문서(「전자정부법」 제2조제7호에 따른 전자문서를 포함한다)로 하되, 청구인이 기명날인(전자서명을 포함한다)하여야 한다. <개정 2010.8.17, 2011.12.8>

1. 청구인의 성명, 주소 및 주민등록번호(외국인인 경우에는 외국인등록번호를 말한다. 이하 같다)
2. 처분을 받은 자의 성명, 주소 및 주민등록번호
3. 심사청구의 대상이 되는 처분의 내용
4. 처분이 있은 것을 안 날
5. 심사청구의 취지 및 이유
6. 심사청구의 연월일
7. 청구인이 처분을 받은 자가 아닌 경우 처분을 받은 자와의 관계
8. 첨부 서류의 표시

A request for review under Article 108 of the Act shall be made by a document (including an electronic document under Article 2, item 7 of the 「Act on Electronic Government」) stating the following matters, and the requester shall affix his or her registered seal (including an electronic signature). <Amended by Act No. 10514, Aug. 17, 2010; Act No. 11137, Dec. 8, 2011> 1. The name, address, and resident registration number of the requester (in the case of a foreigner, the foreigner registration number; hereinafter the same shall apply). 2. The name, address, and resident registration number of the person who received the disposition. 3. The content of the disposition that is the subject of the request for review. 4. The date on which the person became aware of the disposition. 5. The purpose and reasons for the request for review. 6. The date of the request for review. 7. The relationship between the requester and the person who received the disposition, if the requester is not the person who received the disposition. 8. Indication of attached documents.

청구인 및 처분을 받은 자가 가입자 또는 가입자였던 자가 아닌 경우에는 해당 가입자 또는 가입자였던 자의 성명, 주소 및 주민등록번호를 적어야 한다.

청구인의 대리인이 심사청구를 한 경우에는 그 대리인임을 증명하는 위임장을 덧붙여야 한다.
